By booking, I consent to the 24 Hour cancellation policy.

If I no-show or am late to my appointment or I give Perry Massage less than 24 hour notice to move the appointment, and no one books my slot, I understand I will be charged a minimum of 20% to a maximum of 100% of the service price so that Perry Massage can pay their therapist on staff. I consent to Perry Massage holding my card on file and charging me the above amounts deemed fair by the staff if I don’t show for my appointment unless there is a reason beyond the client’s control, such as illness, emergency, death of relative, weather, etc.

If I am late, I understand I will still be charged the full amount of the service.

I consent to Setmore holding my card temporarily, and that I am responsible if my card information is stolen by a third party outside of Perry Massage or if any identity theft occurs because I booked an appointment on a public computer. I am responsible for contacting my credit card company or bank as soon as possible if this occurs to lock my card from future transactions.

I consent to receiving services from Perry Massage and understand that I am responsible for my own well being.

I will not hold Perry Massage, my LMT or any employee/ independent contractor responsible for any accident or damage that happens to myself or parties receiving or affected by services before, during or after my treatment. and agree to forever waive any and all claims and legal rights which I may have whatsoever, to the extent permitted by law, with my use of the Spa facilities.

I agree to abide by all verbal &/or written rules and regulations given to me by the facility. I have not been exposed to anyone with Covid-19 or had it myself in the past 2 weeks.

I understand and agree that massage is a medical service and I will stay covered for the entirety of the service

(except where the therapist has draped and is working).

I consent to my removal from the premises if the professional therapist who is working on me has analyzed and concluded that I am not abiding by the rules of the facility.

I understand Perry Massage will bar from the facility, & report and prosecute to the fullest extent of the law anyone who advances on a therapist, employee or client in the facility in a harmful manner, including unwanted touch, physical or verbal abuse and any sexual harassment by me, the client.
